Information Day - preparing your young person for adulthood
SNAP Parent Carer Forum are offering this FREE information day, suitable for parents/carers whose young person is between 14-25 years of age. The Advocacy & Support Partnership have been invited to to provide this invaluable training.
Themes that will be explored include:
What does the law say for young people with SEN and disabilities, who are approaching 18, or for those who have already reached adulthood?
What rights do parents and carers have in providing, or not having to provide, support?
What care and support are young people aged 18 or over who have additional needs entitled to under the CARE ACT 2014?
What does this mean for young people planning for adult life, and for carers? Or for those already over 18?
Who should be assessed, and what should that assessment look like?
What should a good care plan look like?
How does the care plan link with Education Health and Care Plans (EHCPs)?
What are your rights as carers?
What role does health play in the assessment of care needs?
